Thank you for taking the time to share your information with us. Please do not begin to fill out this form until you and your family have read the entire district update that details the graduation and Class of 2020 Celebration plan. This includes the flowchart.

Information Form Part A is used to gather information for the senior class officers. It will be used to plan events and personalize the Class of 2020 Celebration.

What is your favorite high school memory?

If you are moving away (ex. shipping out for military, moving out of town, moving into college) before the end of summer, please explain and include the date you leave:

Graduation will be happening at some point this summer, even if it is in virtual format in August (refer to the district's mailing and flowchart for better explanation). Still, we understand that some students will not be able to attend a delayed graduation due to various important commitments. Please check this box if you are unable to attend any of the possible dates for graduation and would like to be contacted by school individually about a private graduation ceremony. Even if you may still be able to attend graduation depending on what date it is held, if you check this box, the school will contact you to discuss options in further detail.
